How do I arrange to get COVID vaccination at school?

If you’re a Youth aged 14-17 and currently attend school 

  • You can consent to vaccination on your own.
  • You must book an appointment through Clic-Santé in order to obtain digital proof of vaccination (with a QR code).
  • Youth will receive their proof of vaccination by email after they are vaccinated.
  • Your school will confirm the actual vaccination appointment (day and time).

How to book a vaccination appointment if you’re 14-17 and in school

  1. Go to the Clic-Santé Covid-19 Eeyou Istchee School Vaccination website
  2. Scroll down to your school and click “Continue”.  
  3. Choose a day and time for your appointment and click “Continue”. 
  4. Fill out the information required including your email address. If you don’t include your email address, you won’t receive your QR Code Proof of Vaccination.  

If you are a parent/guardian of a Youth aged 12-13 who currently attends school:

  • You must book an appointment for your child through the Clic-Santé Eeyou Istchee website in order to register them for vaccination, and to obtain digital proof of vaccination (with a QR code).
  • Your school will provide details about the actual vaccination schedule (day and time).
  • Youth aged 12-13 require parent/guardian consent for vaccination. 
  • Your school is distributing consent forms to bring home that require a parent/guardian's signature.

How to book a vaccination appointment for Youth 12-13 in school

  1. Go to the Clic-Santé Covid-19 Eeyou Istchee School Vaccination website
  1. Scroll down to your school and click “Continue”.
  2. Choose a day and time for an appointment and click “Continue”.
  3. Fill out the information required for your child and include your email address or cell phone number.
  1. If you don’t include your email address or cell phone number, you won’t receive your child’s QR Code Proof of Vaccination.

Please Note:

Youth aged 12-17 who are not attending school need to call their CMC for details on when and where they can get vaccinated.
